State your Objective clearly and specifically, rather than generically. Be a star-shaped peg for a star-shaped hole. (We can't help a Generalist.)
If you leave out Duties and Responsibilities in favor of Accomplishments, you will be ahead of the game. If you had a Duty or a Responsibility for which you had no corresponding Accomplishment, then who cares? (We can't help a Caretaker.)
The reverse chronological employment history, showing most recent employment first, should include the following for each position:
 The company name
 Dates of employment
 Your title (include no more than one sentence describing your role)
 One or two sentences telling what business the company was in (not what you did for them)
 Your list of measurable accomplishments and how they impacted the company
